POSITION

This role is a critical team member supporting the overall medical coordination of individuals with intellectual and developmental disabilities. Provides support to the interdisciplinary team in the areas of file management, administrative, and operational functions and serves as a liaison to the medical community.  Works closely with internal nursing and residential staff to support the medical needs of clients in Support, Inc.’s care. 

RESPONSIBILITIES

  • Participate, when appropriate, in the continuing evaluations of the client for purposes of beginning, monitoring, and follow up on an individual's medical progress.
  • Remote work; with the expectation of monthly team and company in person meetings.
  • Schedule, organize, retrieve, review and file medical records, Including the creation and maintenance of Medication Administration Records.
  • Serve as a liaison between the medical care community including primary care providers, emergency services and specialists to gather necessary information for individuals receiving services.
  • Update the interdisciplinary team on medical records for individuals receiving services from medical appointment visit summaries.
  • Create tracking for diet and home exercise plans as prescribed by prescribing physician for individual receiving services in their caseload.
  • Perform basic office duties such as emailing, faxing, printing, copying, scanning, inventory etc.
  • Meet the requirements of all applicable federal and state regulations and Support, Inc. policies and procedures
  • Document, summarize, and communicate, for all relevant parties, each client's status
  • Maintain accepted standards of confidentiality and HIPAA compliance
  • Understand and meet a code of ethics and abide by the Standard of practice relevant to the medical profession
  • Other duties may be assigned.

MINIMUM QUALIFICATIONS

  • Bachelor’s degree or equivalent; or two to four years’ experience and/or training; or equivalent combination of education and experience
  • Must live in Colorado and be able to travel to office for training and meetings. 

PREFERRED QUALFICATIONS

  • Knowledge and experience of physical conditions and treatment protocols for conditions of the IDD population.
  • Strong written and verbal communication skills
  • Comfortability with computers and experience using Outlook, Microsoft Office

SCHEDULE

Monday- Thursday. This position offers a flexible work schedule that may vary based on need.

LANGUAGE SKILLS

Ability to read, analyze and organize various medical directives, professional communication, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, consumers, guardians, family members, and the general public.

REASONING ABILITY

Ability to solve practical and complex problems and critically reason through a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to prioritize work needs to effectively manage time.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to reach with hands and arms and talk or hear. The employee frequently is required to drive. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and ability to adjust focus.
