POSITION SUMMARY

Manages an interdisciplinary team of professionals to achieve high quality service delivery in a community healthcare environment including case coordination, nursing, and clinical staff. Provides leadership and management to continually improve performance, customer outcomes, and growth in services. Ensures that all functions of the department are operating efficiently and effectively and in accordance with regulations, company policy, and best practices. Participates as a member of the Management Team to establish and meet short and long-term strategic goals. This is an exempt, benefit eligible, full-time position (4/10’s: Monday through Thursday) with some flexibility. Salary may depend on a combination of related experience and industry knowledge.

ESSENTIAL DUTIES AND RESPONSIBILITIES

This role will include:

  • Implementation and forward progress of customer care plans
  • Management of staff with varying backgrounds and responsibilities that represent the entire continuum of care for the customer
  • Management of independent contractors working in the field as an extension of the interdisciplinary team
  • Monitors trends in performance using quality assurance methods, such as data trending and key performance indicators
  • Focus on customer relations to retain customers and grow customer base
  • Seeks the opportunity to implement innovation in service delivery
  • Supports and implements processes that seek to improve service delivery and maximize efficiencies
  • Attends the Management Team meetings regularly
  • Reviews analyses of activities, costs, operations, and forecast data to determine department of progress toward stated goals and objectives. Also monitors billing activities for assigned services.
  • Develop and monitor the department’s budget
  • Arranges for recruitment, assignment, and training of appropriate personnel
  • Establishes standards for accountability and consistency of all departmental functions and staff

SUPERVISORY RESPONSIBILITIES

  • Manages a diverse group of staff who also may supervise contractors and other care providers.
  • Carries out supervisory responsibilities in accordance with organization’s policies and applicable laws.
  • Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
  • Must have high integrity, act as a role model for direct reports, and have strong management/leadership skills.

May perform other duties as assigned.

  • Bachelor’s degree required and four to ten years’ related experience in a service delivery environment and/or training; or equivalent combination of education and experience.
  • Master’s degree (M.A.) or equivalent preferred

  • Must be capable of building sustainable systems and processes to improve organizational performance.
  • Willing to grow professionally and enable the growth of individuals around them.
